In order to complete a work permit application before going to the Canadian Embassy at your country of residency, we will need the following information from you, (your family members if applicable):

1. Mailing address (and, if different, residential address)
2. Email address
3. Telephone number
4. Fax number (if applicable)
5. Present marital status (ie: unmarried, engaged, married, widowed, separated, divorced, common-law)
6. Spouse and/or children accompanying applicant? (if any, list all names, dates of birth, places of birth, passport numbers and expiry dates, citizenship, marital status and relationship to applicant)
7. Present job (job title and brief description of position)
8. How long at present job? (months, years)
9. Name and address of current employer, and type of business
10. Date employment with the future employer in Canada is expected to start
11. Date employment with future employer in Canada is expected to end
12. Within the past two years, have you or a family member had tuberculosis of the lung or been in close contact with a person with tuberculosis of the lung?
13. Do you or an accompanying family member have any physical or mental disorder for which that person will require social and/or health services, other than medication, during the stay?
14. Have you or any member of your family ever:
a. committed, been arrested or charged with any criminal offence in any country?
b. been refuesd admission to, or ordered to leave Canada?
c. applied for ANY Canadian Immigration visas (ie: permanent resident, student, worker, visitor, temporary resident)?
d. been refused a visa to travel to Canada?
e. in periods of either peace or war, have you ever been involved in the commission of a war crime or crime against humanity, wuch as: willful killing, torture, attacks upon, enslavement, starvation or other inhumane acts committed against civilians or prisoners of war; or deportation of citizens?
IF YES TO THE ABOVE, YOU MUST PROVIDE DETAILS
15. During the past five years have you or any family member accompanying you lived in any other country than your country of citizenship or permanent residence for more than six months?
